Megan Fox is turning heads in a new advertising campaign for healthcare company Ro, but the actress’ appearance quickly became just as much of a talking point as the product she was promoting.
The Jennifer’s Body actress stars in the new campaign for Ro Sparks, a compounded, two-in-one prescription medication used to treat erectile dysfunction (ED). In the ad, she takes on the persona of Ro Sparks while sporting a bold, form-fitting look.
For the campaign, the mom of four wears a skin-tight red catsuit featuring the Ro Sparks logo, pairing the outfit with matching red pumps. She wears her dark hair loose in a center part, while her signature beauty look includes defined dark eyebrows, winged eyeliner and a glossy plum-colored lip.

Megan Fox Fans React To Her 'Different' Appearance
The sultry campaign immediately caught fans’ attention, with some commenters saying they were struck by how different Fox appeared compared with earlier stages of her career.
“You don’t look like Megan Fox anymore,” one person wrote.
“She looks different. Still love her tho,” another commenter said.
Someone else speculated about cosmetic procedures, writing, “She’s overdone it with the fillers a bit. I was thinking that the whole time watching this.”
“Who is this?” another person asked.
In 2024, during an appearance on the Call Her Daddy podcast, Fox opened up about some of the cosmetic procedures she has undergone, including breast augmentation.

Megan Breaks Down Her Cosmetic Surgeries
The 40-year-old revealed that she first got breast implants when she was around 21 or 22 and has had the procedure three times in total.
She explained that she had her implants replaced after breastfeeding her children, saying, “I had them redone after I was done breastfeeding my kids because I don't know where they went, but they went.”
She later had another replacement because she did not have enough body fat to conceal the implants, which caused visible rippling. Fox has also acknowledged using injectables, including Botox and dermal fillers.
However, she said the only other major cosmetic surgery she has undergone was a rhinoplasty.
“I had my nose done when I was in my early twenties,” she said, adding that people often assume she has had multiple nose jobs.
“I haven't had a rhinoplasty since I was, I'm going to say, 23. It's been well over a decade. I've not touched my nose since then,” Fox said.
